Account owner, trustee or custodian name changes

To update your name on most account types:

  1. Complete the Name Change Request (PDF). You must do one of the following:
    • Have your former and new name validated in section 2 of the form
    • Provide a legal name change document (e.g. marriage certificate, divorce decree, legal name change certificate)
       
  2. Send us the completed form and, if applicable, a copy of the name change document, via one of the following:
